Voucher Terms and Conditions:
1. Eligibility: Vouchers are available to all existing patients of Smile Suffolk Dental and Implant Clinic who refer a new patient (family member or friend) to our clinic. The referral must be an adult new patients who has not seen in clinic in last 2 years.
2. Referral Qualification: The new patient must schedule and complete their first appointment at our clinic to qualify for the referral voucher.
3. Voucher Value: Both the referrer (existing patient) and the referred patient will receive a voucher valued at £10 that is redeemed on any spend at smile Suffolk.
4. Voucher Redemption: Vouchers can be redeemed on their next visit to Smile Suffolk Dental and Implant Clinic. Vouchers are not transferable or exchangeable for cash.
5. Expiration: Vouchers are valid for 24 months from the date of issue. Please check the expiration date on the voucher.
6. Multiple Referrals: Existing patients can refer multiple new patients and receive a £10 voucher for each successful referral.
7. Notification: Vouchers will be issued in-person notification to the referrer and the referred patient.
8. Clinic Discretion: Smile Suffolk Dental and Implant Clinic reserves the right to modify or terminate this voucher program at any time without prior notice.
9. Fraud Prevention: Any attempt to manipulate or misuse the referral program, including false referrals, may result in disqualification.
